What is the purpose of the survey?

This survey is your opportunity to provide feedback on what you value about QCOSS and what you believe could be improved. Your responses will feed into the strategic and operational planning process to help guide the focus of QCOSS.

We are keen to hear your views and thoughts. There are no right or wrong answers. Positive and negative answers will provide valuable insight into how you view QCOSS and are very welcome.

We encourage you to complete the survey so we can better understand how we can best support equality, opportunity and wellbeing for every person in every community.

Confidentiality:

All individual information that could possibly identify you will be held and (after a given time) destroyed by the independent evaluator – Ardjuna. A general report will be released at the end of the survey. This general report will only present aggregated data and your comments will only be presented in summarised format.

Your comments and demographic details will be made available to the CEO in a special executive report that will not be released for general consumption.

If you wish the CEO to respond to you about an issue you raise in completing the survey, please clearly say so in your response. The evaluator will forward that specific information to the CEO. All other responses will be confidential.
