Request Form (Rockhampton Principal)

This form gathers all the information necessary for creating a performance appraisal for a specific person or a review of some other specific target (e.g. school).

If you are setting up a performance appraisal for a specific person, there is a hardcopy of this form available for download by clicking here. You can print this form and use it to collect the necessary detailed appraisal information during the preparation meeting/process. If you are setting up an online survey for something else other than a role appraisal (e.g. review of a school), there is a version of the hardcopy form specifically design for that purpose. Before completing the form below it is important to do the following:

1. Identify, revise or develop the question bank that you want to use for this role appraisal or review of something else (e.g. school). You can review the question bank development process by clicking here. If you have already developed a question bank for this type of appraisal or online survey, you can check that it has been set up in the question bank database by looking for the name of the question bank in the drop down list below (see please select the question bank…).

2. Identify the various response groups that you want to target (total of 8 response groups are available including the person being appraised if the process is a 360 degree performance appraisal process). The response groups should have been identified during the preparation process. Please remember that you if you have more than 8 response groups, you need to think about how best to merge some response groups in order to produce the best possible final report).

3. Develop the mailing lists for each of the identified response groups (e-mail, physical addresses for both). Click here to access resources for developing the mailing lists.

4. Decide whether your organisation, Ardjuna or both parties are going to manage the process of mailing out the invitation to participate letters and reminders to members of the various response groups.Please note that Ardjuna do not handle mail-out of hardcopy surveys within the cost structure of the 360 degree online process. If the Project Manager is going to manage the mailing of invitation letters and reminders to all, some or part of the response groups then click here to access resources for developing the mailing lists.

5. Secure any customized questions that will be added to the question bank for this particular appraisal/online survey

Response Groups

Please remember that the online survey process only accommodates 8 response groups. In the case of a 360 degree performance appraisal, the first response group is allocated to the person being appraised. If you have more than 8 targeted response groups, you need to think about how best to merge some groups in order to provide the best possible final report. If you choose to merge various response groups, please select the "other" option from the drop-down list, and then describe which groups have been merged e.g. assistant principal and school board. If the name of a response group is not included in the drop down list please select the “other” option and insert a description of the response group in the box provided. Please note that Ardjuna do not handle mail-out of hardcopy surveys within the cost structure of the 360 degree online process.

Please use the template for developing the response group mailing lists that the Project Manager is responsible for managing (unless you already have a spreadsheet available).

Once you have compiled the mailing lists, please e-mail the excel spreadsheet to Ardjuna (auto@arduna.com). This information will be stored on a protected server. As soon as the information is no longer needed it will be permanently deleted.

Tip about developing questions for performance appraisals When developing school/situational specific questions for a performance appraisal, it is important that the questions make sense from the perspective of the person being appraised and the people appraising him/her. We therefore recommend that you use the crossover format made up of a verb the spiritual mission of the school, Settles disputes between staff effectively, Is often overwhelmed etc.). It is also important to avoid double barrelled questions (e.g., Is available for discussing new ideas and listens well). If you are developing a survey for something else like securing feedback about the performance of a school as a whole, then the question format is more flexible.
