A question bank is needed to conduct an online survey. A question bank for a particular role or other target (e.g. school) is established by - (1) using an existing question bank without revision, (2) selecting and amending an existing question bank, or (3) developing a new question bank. When making a request for a specific online survey to be set up (e.g. appraisal of a specific person, school, spending proposal etc.), you will be asked to identify which question bank you intend to use. It is therefore important that you either identify an existing question bank that you intend to use without revisions, or you need to revise/develop and submit your preferred question bank prior to making a request for a specific online survey. In order to determine whether you would like to use an existing question bank with or without revisions, we suggest that you review the existing database of sample question banks by clicking here. If you are going to revise an existing question bank or develop a new one, it is important that you review the design requirements and restrictions for creating surveys by clicking here.

Use an existing question bank with some revisions to the content for use with a new target (e.g. use the existing Brisbane Archdiocese principal appraisal question bank with some revisions to appraise principals in the Cairns Diocese)

1. Use an Existing Question Bank without Revisions for a New Target


If you intend to use an existing question bank without revisions for a new target (e.g. use the existing Brisbane Archdiocese principal appraisal question bank to appraise principals in the Cairns Diocese) then you still need to complete the question bank development process so that a version is created for the Cairns Diocese. In order to do this, please ensure that you complete the following key components of the question bank development process:

a. select the existing question bank from the drop down list

b. write a description of what the new target is (e.g. principals in the Cairns Diocese)

c. submit the question bank

Ardjuna.com will then enter the question bank into the database of active question banks using a title that reflects the target and associated (Arch)Diocese. The title is based on the following naming convention (Diocese-Target-Version Date) for example, Brisbane-School Principal-Feb2010 or Cairns-Primary Schools-Mar2011 or Bathurst-St Patricks Expansion Proposal-June2010). Once a question bank has been added to the database it shall remain there for future use unless Ardjuna.com is instructed to remove it. The question bank can now be used in the future without having to complete the question bank development process (as long as no revisions are required).

2. Revise an Existing Question Bank for the Same Target

If you would like to revise the content of an existing question bank for the same target (e.g. revise some of the content in the existing Brisbane Archdiocese principal appraisal question bank) then you still need to complete the question bank development process so that a new version can be created. In order to do this, please ensure that you complete the following key components of the question bank development process:

a. select the existing question bank from the drop down list

b. write in the description box that this is a new version of an existing question bank for use with the same target (e.g. updated version of the primary school survey for schools in the Bathurst Diocese – for use with the same target)

c. amend the content that appears in the category and question boxes as necessary (please ensure that you adhere to the design requirements and restrictions of the survey – to review design requirements click here)

d. submit the question bank

The project manager will conduct a review of the question bank and help you make any necessary revisions to ensure that the changed content will create an effective survey. The question bank will then be entered into the database of active question banks using a title that reflects the target and associated (Arch)Diocese. The title is based on the following naming convention (Diocese-Target-Version Date) for example, Brisbane-School Principal-Feb2010 or Cairns-Primary Schools-Mar2011 or Bathurst-St Patricks Expansion Proposal-June2010). Once a question bank has been added to the database it shall remain there for future use unless Ardjuna.com is instructed to remove it. The question bank can now be used in the future without having to complete the question bank development process (as long as no revisions are required).

3. Revise an Existing Question Bank for a New Target

If you wish to use an existing question bank for a new target, and make revisions to some of the content, then you need to complete the question bank development process so that a different question bank is created. In order to do this, please ensure that you complete the following key components of the question bank development process:

a. select the existing question bank from the drop down list

b. write a description of what the new target is (e.g. deputy principals in the Brisbane Archdiocese)

c. amend the content that appears in the category and question boxes as necessary (please ensure that you adhere to the design requirements and restrictions of the survey – to review design requirements click here)

d. submit the question bank

The project manager will conduct a review of the question bank and help you make any necessary revisions to ensure that the changed content will create an effective survey. The question bank will then be entered into the database of active question banks and will be given a title based on the following naming convention (Diocese-Target-Version Date) for example, Brisbane-School Principal-Feb2010 or Cairns-Primary Schools-Mar2011 or Bathurst-St Patricks Expansion Proposal-June2010). Once a question bank has been added to the database it shall remain there for future use unless Ardjuna.com is instructed to remove it. The question bank can now be used in the future without having to complete the question bank development process (as long as no revisions are required).

4. Develop a New Question Bank

If you wish to develop a new question bank then please complete the question bank development process as follows:

a. provide a description of the new question bank you are developing (e.g. this is a new question bank that will be used to review primary schools in the Northern District of the Cairns Archdiocese)

b. enter the category descriptions and questions into the space provided (type in the content or copy/paste) (please ensure that you adhere to the design requirements and restrictions of the survey – to review design requirements click here)

c. submit the question bank

Please remember that you have a maximum of 14 categories with a maximum of 5 questions per category, and we prefer that each active category contain no less than 5 questions. The project manager will conduct a review of the question bank and help you make any necessary revisions to ensure that the content will create an effective survey. The question bank will then be entered into the database of active question banks and will be given a title based on the following naming convention (Diocese-Target-Version Date) for example, Brisbane-School Principal-Feb2010 or Cairns-Primary Schools-Mar2011 or Bathurst-St Patricks Expansion Proposal-June2010). Once a question bank has been added to the database it shall remain there for future use unless Ardjuna.com is instructed to remove it. The question bank can now be used in the future without having to complete the question bank development process (as long as no revisions are required).

Tip about developing questions for performance appraisals

If you are developing a performance appraisal question bank, it is important that the questions make sense from the perspective of the person being appraised and the people appraising him/her. We therefore recommend that you use the crossover format made up of a verb and target (e.g. Ensures compliance with school policy, Is supportive of distressed parents, Promotes the spiritual mission of the school, Settles disputes between staff effectively, Is often overwhelmed etc.). It is also important to avoid double barrelled questions (e.g., Is available for discussing new ideas and listens well). If you are developing a survey for something else like securing feedback about the performance of a school as a whole, then the question format is more flexible.
